Some forms, such as lost license plates, handgun permit applications or background checks/fingerprinting require you to purchase these forms for a nominal fee.

Motor vehicle inspections forms for BMV are available for purchase for $5.00 also from our Records Bureau if brought to our stattion (additional cost for sending officer out)
